Why does a lower denture fit so much more poorly than an upper denture?
Good question.... but it is a very frustrating question for both patients and dentists in Highlands Ranch as well as patients and dentists through out the country. A lower denture has so many potential problems that include:
- A thin skinny ridge in which to saddle the lower denture to... not a big broad palate that is seen in the upper denture
- The tongue moves and this movement can contribute to the lack of suction of the lower denture.
- The skinny bony ridge and gum that the lower denture sits on shrinks (resorbs) three (3x) faster than the upper (maxillary) ridge. The Mandibular ridge melts away quickly and has less and less support with time.
- With time... there is little/no ridge to ":saddle" the denture to
- As the lower ridge shrinks away... the bone can become so thin that the lower jaw (mandible) can fracture quite easily.
- Teeth (and dental implants) act to stimulate the bone of the jaws especially in the mandible.
- When teeth are removed, the stimulus to keep the bone is gone and the bone resorbs. This is why our Highlands Ranch dentists try to have patients either keep their teeth as long as practical or ... have implants placed.
- Dental mplants are placed in the lower jaw (mandible) for at "least" two reasons
- To act as a stabilizing factor for resistance and retention of the denture.
- Help reduce the bone resorption found when teeth are removed... most critical on the lower (mandibular) ridge.
Good fit and retention (in general)
- Complete implant supported denture tooth by tooth
- Implant supported denture with more dental implants.
- Implant supported denture a few implants
"Fair" fit (at best)
- Maxillary denture (upper denture)
Poor/hopeless fit
- Mandibular (lower denture)
